02 Passport having validity of at least (06) six months from the date of application. Copy of the latest passport of first six pages. If one brings previous passports, references of previous Passport should be mentioned in the new passport.
03 (04) Four passport size color photograph (Background must be white)
04 Detail letter of introduction addressed to the Embassy by the applicants' company (even for the owner). For the employee, original appointment letter with one photocopy set, original salary statement of last six months with one photocopy set.
05 If available, original and one copy of registration documents of the applicants' company, Certificate of incorporation, memorandum and articles of association (copies of the first two and last one page with the names of the shareholders),valid membership certificate with local chamber of commerce, tax number, VAT registration, trade license, any other important business documents.
06 Letter of credits (if available), any documents about previous business with Italian companies. (Original with Photocopy)
07 A detail invitation of the Italian company must be sent directly to the Embassy before the applicants come to Embassy.
08 Bank solvency, Last (06) months Bank statement with Bank forwarding letter (Company accounts & personal account. International Credit Card (if available, Travelers cheque).
09 Travel Health Insurance (Medical) with a coverage of at least 30000/=Euro or more for the period of visit (Original with photocopy).
10 Hotel reservation for the duration of stay at Italy. (Original with photocopy)
11 Flight booking. (Only PNR)

Visa applicants have to come personally to the Visa Section of the Embassy and bring the

04. A detail invitation including what kinds of cost will be provided by them must reach the Embassy before the applicants come to Embassy.
05. Details letter of introduction addressed to the Embassy by the applicants' Company or organization. Original appointment letter with one photocopy set, original salary statement of last six months with one photocopy set.
06. Bank solvency, Last (06) months Bank statement with Bank forwarding letter (Company accounts & personal account. International Credit Card (if available, Travelers cheque).

MEDICAL CARE VISA FOR ITALY
Required Process / Documentation


IN ORDER TO OBTAIN A MEDICAL VISA A FOREIGNER WHO WOULD BEAR THE ENTIRE COST OF THE MEDICAL CARE AND TREATMENT IN ITALY, NEEDS THE FOLLWING DOCUMENTS.

1. A Declaration issued by the recognized public or private medical institution that has been chosen, in which it is stated the kind, the starting date and the approximate duration of the medical treatment. (Declarations from not recognized private institutions will not be considered as valid)

2. A Certificate that attests that an amount of money corresponding to the presumed cost of medical expenses has been deposited as security. The deposit in EURO, or US Dollars must correspond to the 30% of the presumed total cost of the treatment and must have been deposited in the chosen medical institution.

3. Necessary documentation that proves the availability of sufficient financial resources in Italy in order to bear the total cost of medical treatment, board and lodging outside the medical institution, own repatriation and of an eventual assistant.

Those who are interested in receiving the above-mentioned Visa must go in person to the Visa Section of the Italian Embassy producing the following.

Appropriately filled up and signed Visa Form

Passport with expiry date at least six moths longer than the required Visa

Two passport size photographs

Flight ticket appropriately booked. Before colleting the required Visa a confirmed, not repayable return ticket must be produced.

To note:
The Embassy may ask for additional documentation to the above mentioned ones.

STUDY VISA FOR ITALY
Required Process / Documentation

A Study Visa is issued for :

University studies;

Academic study or professional formation's courses at recognized and qualified institutes;

Research and cultural activities.

Necessary Requirements

1. Studies, professional formation or cultural activities that are going to be undertaken must have documented guarantee;

2. The guarantee about self-maintenance must be adequate and not lower than the annual social benefit related to the duration of the required stay;

3. Insurance policy for medical care and hospital admission that may cover the entire duration of the permanence in Italy;

4. The candidate must be older than 14 years old.

To note:
The Embassy may ask for additional documentation to the mentioned one.
