Required Process / Documentation
IN ORDER TO OBTAIN A MEDICAL VISA A FOREIGNER WHO WOULD
BEAR THE ENTIRE COST OF THE MEDICAL CARE AND TREATMENT
IN ITALY, NEEDS THE FOLLWING DOCUMENTS.
1. A Declaration issued
by the recognized public or private medical institution
that has been chosen, in which it is stated the kind,
the starting date and the approximate duration of the
medical treatment. (Declarations from not recognized
private institutions will not be considered as valid)
2. A Certificate that
attests that an amount of money corresponding to the
presumed cost of medical expenses has been deposited
as security. The deposit in EURO, or US Dollars must
correspond to the 30% of the presumed total cost of
the treatment and must have been deposited in the chosen
medical institution.
3. Necessary documentation
that proves the availability of sufficient financial
resources in Italy in order to bear the total cost of
medical treatment, board and lodging outside the medical
institution, own repatriation and of an eventual assistant.
Those who are interested in receiving
the above-mentioned Visa must go in person to the Visa
Section of the Italian Embassy producing the following.
Appropriately filled up and signed Visa Form
Passport with expiry date at least six moths longer
than the required Visa
Two passport size photographs
Flight ticket appropriately booked. Before colleting
the required Visa a confirmed, not repayable return
ticket must be produced.
To note:
The Embassy may ask for additional documentation to
the above mentioned ones. |
