Visa Application "FORM' must be filled in, dully with dated and signed. For minors (if they
accompany their father or mother or both), both parents should sign the forms and if only
Father or Mother wants to bring his or her minor son or daughter to Italy, No Objection
Certificate from father or mother who is not accompanying will be needed along with his
Passport and its photocopy. |

Passport having validity of at least (06) six months from the date of application. Copy of the
latest passport of first six pages. If one brings previous passports, references of previous
Passport should be mentioned in the new passport. |

(04) Four passport size color photograph (Background must be white) |

Detail letter of introduction addressed to the Embassy by the applicants' company (even for
the owner). For the employee, original appointment letter with one photocopy set, original
salary statement of last six months with one photocopy set. |

If available, original and one copy of registration documents of the applicants' company,
Certificate of incorporation, memorandum and articles of association (copies of the first two
and last one page with the names of the shareholders),valid membership certificate with local
chamber of commerce, tax number, VAT registration, trade license, any other important
business documents. |

Letter of credits (if available), any documents about previous business with Italian companies.

A detail invitation of the Italian company must be sent directly to the Embassy before the
applicants come to Embassy. |
| 08 |
Bank solvency, Last (06) months Bank statement with Bank forwarding letter (Company
accounts & personal account. International Credit Card (if available, Travelers cheque). |

Travel Health Insurance (Medical) with a coverage of at least 30000/=Euro or more for the
period of visit (Original with photocopy). |

Hotel reservation for the duration of stay at Italy. (Original with photocopy) |
| 11 |
Flight booking. (Only PNR) |

Please Note: Visa applicants might be request to produce additional documents to the ones listed above.
 Visas are generally issued after minimum 10 working days from the moment the application has been
accepted.
Usually original bank statements and letter of presentation are received and copies of other documents
are received giving back their original ones after checking. |
